The above form has the line: "Next Rabies Vaccination On:" . If you are going to print your own medical exemption form, then you might not want that line in it. We have modified the form by removing that line and made it available on VacLib.org.

Abstract: (1) All dogs and cats 4 months of age or older must be vaccinated by a licensed veterinarian against rabies with a United States Government-approved vaccine.
(2) A dog or cat is exempt from vaccination against rabies if a licensed veterinarian has examined the animal and has certified in writing that at the time vaccination would endanger the animal's health because of its age, infirmity, disability, illness, or other medical considerations. An exempt animal must be vaccinated against rabies as soon as its health permits.

AB-527 (Medical Exemption to the Rabies Vaccine) was signed by Governor Doyle (Thursday) the 30th of March. (2006)
Assembly Bill 527 authorizes a city, village or town to exempt the owner of a dog from the requirement to have the dog vaccinated against rabies if the owner provides a letter from a veterinarian stating that vaccination is inadvisable for reasons related to the dog's health. The bill helps protect canine health by allowing veterinarians to withhold a rabies vaccination when medical conditions warrant.
